Đối với phụ gia yêu cầu nhà cung ứng, nhà sản xuất phải có một trong các chứng nhận trên hoặc có chứng nhận FAMI-QS.
Đối với nhà cung ứng mới: Tiêu chí đánh giá được qui định trong phiếu đánh giá nhà cung ứng mới.
Đối với nhà cung ứng truyền thống: tiêu chí được quy định trong các phiếu đánh giá sau: phiếu đánh giá giám sát nhà cung ứng nguyên – phụ liệu và vật tư thiết bị, phiếu đánh giá giám sát dịch vụ vận chuyển, phiếu đánh giá giám sát nhà cung ứng dịch vụ thử nghiệm.
c) Lập danh sách nhà cung ứng ban đầu
Cập nhật tất cả nhà cung ứng truyết thống và nhà cung ứng mới (có kết quả chấp nhận hoặc tạm chấp nhận) vào danh sách nhà cung ứng được đánh giá, người đánh giá phải thu thập đầy đủ thông tin theo biểu mẫu danh sách nhà cung ứng được đánh giá.
d) Tiến hành đánh giá
 Đối với nhà cung ứng mới
Tiến hành đánh giá nhà cung ứng mới theo biểu mẫu, đánh giá cả chất lượng mẫu chào hàng (nếu có) hoặc chất lượng lo hàng đầu tiên.
Sau khi nhà cung ứng mới được đánh giá “chấp nhận” hoặc “tạm chấp nhận” thì sau cuối mỗi quí ta tiến hành đánh giá giám sát nhà cung ứng nguyên – phụ liệu và vật tư thiết bị. Trường hợp “không chấp nhận” ngưng ký tiếp mẫu chào hàng khác, khi đó tiến hành đánh giá lại.
Ghi chú: kết quả đánh giá tiêu chí “chất lượng hàng hóa” được căn cứ vào kết quả đánh giá của Phòng Công Nghệ Chất Lượng.
Tổ chức tham quan nhà xưởng và đánh giá trực tiếp cơ sở vật chất, giấy phép thành lập kinh doanh và khả năng của nhà cung ứng (nếu cần).
 Nhà cung ứng truyền thống
Tiến hành đánh giá nhà cung ứng truyền thống trên phiếu đánh giá giám sát nhà cung ứng nguyên – phụ liệu và vật tư thiết bị.
e) Lập danh sách nhà cung ứng chính thức
Danh sách nhà cung ứng được chọn phải lập theo thứ tự từ kết quả đánh giá theo biểu mẫu danh sách nhà cung ứng chính thức.
f) Phê duyệt
Ban Giám Đốc sẽ xem xét tiêu chuẩn đánh giá nhà cung ứng, đồng ý sẽ phê duyệt cho lập danh sách các nhà cung ứng được chọn.
g) Lưu hồ sơ
Danh sách nhà cung ứng được chọn sẽ được cập nhật thường xuyên, phải duy trì hồ sơ các kết quả đánh giá và mọi hành động cần thiết phát sinh từ việc đánh giá.
5.8. Đánh giá giám sát nhà cung ứng
Đánh giá nhà cung ứng nguyên – nhiên – phụ liệu và dịch vụ vận chuyển theo phiếu đánh giá giám sát nhà cung ứng nguyên –nhiên – phụ liệu sau thời gian 3 tháng/lần. Đánh giá nhà cung ứng dịch vụ thử nghiệm và vật tư thiết bị sau thời gian 6 tháng/lần. Hoặc đánh giá đột xuất theo yêu cầu của Ban Giám Đốc. Công Ty tiến hành đánh giá giám sát nhà cung ứng có tên trong danh sách nhà cung ứng chính thức và vẫn đang cung cấp hàng hóa và dịch vụ cho Công Ty.
